Required documents for Foreign nationals
- Photocopy of a valid passport (Identification/Photo page)
- Photocopy of your Visa which is issued by Thailand Immigration or Thai Embassies or Consulates
- Photocopy of the last visa stamp in your passport (for entry into Thailand)
- A photocopy of the Thai working permit (if available)
- Two recent photographs 1 – 2 inches with white background
- Two sets of the original fingerprint
If you live Outside of Thailand: Fingerprints must be collected by the local police or the Thai Consulate or Thai Embassy in your country.
If you live in Thailand: Fingerprint must be collected in person at the Royal Thai Police head office in Thailand.

เอกสารประกอบในการยื่นคำขอออกหนังสือรับรองความประพฤติ
- สำเนาหนังสือเดินทาง (Passport) ที่ยังไม่หมดอายุ (มีอายุเหลือ 6 เดือนขึ้นไป และสำเนาต้องเห็นรูปถ่ายชัดเจน)
- สำเนาบัตรประจำตัวประชาชน
- สำเนาทะเบียนบ้าน
- สำเนาหลักฐานการเปลี่ยนชื่อ นามสกุล (ถ้ามี)
- สำเนาใบสำคัญการสมรส/หย่า (ถ้ามี)
- รูปถ่ายขนาด 1-2 นิ้ว จำนวน 1 ภาพ
- สำหรับชายไทย อายุตั้งแต่ 17-45 ปี ต้องมีหลักฐานทางการทหารของชายสัญชาติไทย (สด. 8, สด.9, สด. 43, ใบรด., หนังสือผ่อนผันทหาร ฯลฯ )
>> ถ้าอายุเกิน 45 ปี ไม่ต้องใช้
